Travel insurance for residents of Spain: what to check before a trip
Living in Spain changes your travel cover. What the EHIC (TSE) covers, why home-country policies may fail, and what to check before you go.
When you live in Spain, travel insurance works a little differently from when you lived in your home country. The policy you used to buy back home may no longer cover you, your Spanish public health cover only goes so far abroad, and your private health insurance may or may not include travel. With Christmas trips and winter holidays coming up, this guide explains what to check before you travel, whether you are flying home for the holidays, going skiing or visiting another country.
Your old home-country policy may not cover you
Most travel insurers sell their policies only to people who live in the country where the insurer operates, and they define “living there” in their terms. As an example, one UK insurer requires customers to have lived in the UK for six of the last twelve months, to be registered with a UK doctor and to travel from and return to the UK. Someone who has moved to the Costa del Sol will usually not meet conditions like these.
If you buy a policy that requires residence in another country and you no longer live there, the insurer may refuse a claim. The safe approach is to buy cover that is meant for residents of Spain, and to tell the insurer honestly where you live. This applies to annual policies you renew automatically too, so check your old ones.

What the European Health Insurance Card covers
If you are covered by the Spanish public health system through the Seguridad Social, you can apply for a European Health Insurance Card, known in Spain as the Tarjeta Sanitaria Europea (TSE). According to the Spanish government, the card:
- Is free and is issued by the Seguridad Social. You can apply online, by phone or at a CAISS office. Each traveller needs their own card.
- Arrives by post at the address the Seguridad Social has on file, normally within about five days.
- Is generally valid for two years. For foreign residents, it ends when the residence permit ends.
- Works in the 27 EU countries, the UK, Norway, Iceland, Liechtenstein and Switzerland.
Just as important is what it does not cover: private healthcare, rescue and repatriation, and planned treatment abroad. In practice, the card gives you access to public healthcare in the country you visit on the same terms as local people, which may include paying part of the cost. It is not a replacement for travel insurance. If you need to travel before the card arrives, you can request a provisional replacement certificate (Certificado Provisional Sustitutorio), which is valid for 90 days.

Does your private health insurance cover travel?
Many residents have private health insurance in Spain, sometimes because it was required for their visa. Some of these policies include travel assistance abroad (asistencia en viaje), but often with limits on the number of days per trip, the countries covered or the amount paid. Others cover only treatment in Spain.
Look in your policy conditions for a section on travel assistance, or ask your insurer directly in writing. Our guide to insurance in Spain explains the main types of cover and the Spanish terms you will see.
What to check in a travel policy
- Medical expenses: the maximum amount paid for treatment abroad. Trips outside Europe, especially to the United States, need much higher limits.
- Repatriation: whether the policy pays to bring you back to Spain if you fall ill or are injured, and whether it brings you back to Spain or to another country.
- Pre-existing conditions: many policies exclude illnesses you already had when you bought the policy unless you declare them. Declare everything you are asked about.
- Cancellation: what counts as a valid reason to cancel, such as illness or a death in the family, and the maximum refund.
- Activities: skiing and other winter sports are often excluded or need extra cover.
- Excess (franquicia): the amount you pay yourself on each claim.
- Single trip or annual: if you travel several times a year, an annual policy can be simpler. Check the maximum length of each trip.
- Language and contact: make sure the 24-hour assistance line takes calls in a language you speak, and save the number in your phone.
Insurance included with bank cards
Some bank cards, mainly credit cards, include travel insurance or travel assistance when you pay for the trip with the card. The cover varies a lot between cards and is sometimes limited to accidents or delayed baggage. Read the card’s insurance conditions on your bank’s website and check whether you must pay the whole trip with the card for the cover to apply.
Delays, cancellations and lost luggage
For flights, you have rights under EU air passenger rules regardless of your travel insurance, for example when a flight from an EU airport is cancelled or badly delayed. Claim first from the airline. In Spain, the Agencia Estatal de Seguridad Aérea (AESA) handles complaints if the airline does not respond or you disagree with its answer. For lost or damaged luggage, report it at the airport before you leave the baggage hall and keep the report. Your travel insurance may then cover what the airline does not.
Before you leave: a short checklist
- Check that your TSE is valid and has not expired with your residence permit.
- Confirm that your travel policy is for residents of Spain and covers your destination and activities.
- Save the policy number and the assistance phone number on your phone and on paper.
- Take a photo of your passport, residence card and insurance documents, and keep it somewhere safe online. If your passport goes missing, our guide on lost or stolen passports explains what to do.
FAQ: travel insurance for residents of Spain
Can I keep using a travel policy from my home country?
Often not. Many insurers only cover people who live in their country. If you have moved to Spain, check the residency conditions in the policy, and buy cover meant for residents of Spain if you no longer meet them.
Is the European Health Insurance Card enough for a trip in Europe?
It gives access to public healthcare on the same terms as locals, but it does not cover private care, rescue or repatriation. Travel insurance is still recommended.
Does the Spanish EHIC work in the UK?
Yes. The Spanish government lists the UK among the countries where the card can be used, together with the EU, Norway, Iceland, Liechtenstein and Switzerland.
Do I need travel insurance for trips within Spain?
Your Spanish public or private health cover applies at home, but travel insurance can still be useful for cancellation, luggage or activities such as skiing. Check what your existing policies already cover.
